Diagnostic Testing for Carpal Tunnel Syndrome
Carpal Tunnel Syndrome (CTS) is a condition that results from compression of the median nerve within the carpal tunnel of the wrist. Numbness and tingling of the thumb, index finger and middle fingers are common complaints. Early and accurate diagnosis of CTS is critical for planning effective treatment modalities.
A detailed history and a physical examination are easily performed in an office setting.
The value of Diagnostic Testing in CTS was recently presented in an article in the Journal of Hand Surgery...(Szabo, RM et al, Journal of Hand Surgery 1999; 24A: 704-714). The authors found that in patients with abnormal hand diagrams, a positive Durkan CTS test, abnormal sensibility by Semes-Weinstein testing and night pain, the probability the CTS will be diagnosed is 0.86. If all 4 of these conditions are normal, the probability of that the patients have CTS is 0.0068. The addition of electrodiagnistic testing did not increase the diagnostic power of the combination of these 4 clinical tests. Click here to view the abstract from this article.

The illustration on the right shows the median nerve passing through the flexor retinaculum on the palm side of the wrist. The carpal tunnel is formed by this retinaculum on the roof and the bones of the wrist on the remaining three sides. Flexor tendons of the wrist, thumb and four fingers also pass through the carpal tunnel.
